Informed Consent and Release for Permanent Makeup (PMU) Services

Client and Artist Information

1. Nature and Process of Permanent Makeup

Permanent makeup (PMU), also called micropigmentation or cosmetic tattooing, is a procedure where pigments are injected into the upper layers of the dermis to enhance facial features (such as eyebrows, lips, or eyelids). The process involves: (1) Consultation and mapping: drawing the shape outline to agree on symmetry and placement. (2) Topical numbing: applying a numbing cream to the skin to minimize discomfort. (3) Pigment application: using manual blades (microblading) or a digital rotary machine with sterile, single-use needles to insert pigment. (4) Cleaning and aftercare dressing. The procedure causes minor bleeding and moderate discomfort. PMU is considered semi-permanent and will fade over 1 to 3 years, requiring maintenance touch-ups.

2. Optional Pigment Patch Test

[ ] I request a pigment patch test today. A small amount of pigment will be applied to my skin (behind the ear) to check for allergic reactions. I understand this requires waiting 24 to 48 hours before the full procedure can be performed.
[ ] I decline the pigment patch test and authorize the artist to proceed with the full treatment. I accept the risk of any allergic reaction to the pigments used.

3. Health Screening and Disclosures

[ ] I am NOT pregnant or breastfeeding.
[ ] I have NOT used oral Accutane (isotretinoin) in the past 12 months.
[ ] I do NOT have a history of keloids or abnormal scarring.
[ ] I have disclosed any history of cold sores (especially if Lip Blush is planned). Lip tattooing can trigger herpes simplex outbreaks, and antiviral medication is recommended beforehand.
[ ] I do NOT have glaucoma, dry eye syndrome, or wear eyelash extensions (if eyeliner PMU is planned).

4. Short-Term Risks and Side Effects

Swelling and redness: expected around the treated areas. Swelling is usually mild for eyebrows but can be moderate for lips and eyeliner, resolving in 24 to 72 hours.
Scabbing and peeling: the treated skin will form dark scabs as it heals. The pigment will appear very dark for the first 3 to 7 days, then peel away, revealing a lighter, softer color.
Local infection: bacteria entering the healing skin. Signs include spreading redness, warmth, throbbing pain, or yellow crusting.

5. Long-Term and Permanent Risks

Color shifting or fading: pigments can fade or shift color (e.g., turn blue/grey, red/pink, or orange/brown) over time due to sun exposure, skin chemistry, or skincare products (retinoids, acids).
Asymmetry: minor asymmetry in shape or placement can occur as the skin heals or if facial muscles move unevenly.
Allergic reaction to pigments: rare, but can develop years after the procedure, causing itching or bumps.
Permanence: although called semi-permanent, residual pigment can remain in the skin permanently.

6. Alternatives to Permanent Makeup

Standard daily cosmetics (eyebrow pencils, lipsticks, eyeliners) which are non-invasive and temporary.
Henna brow tinting or eyebrow laminations, which last 2 to 6 weeks.
No treatment: choosing not to modify the facial skin.

7. Detailed Aftercare Guidelines

To protect your PMU and ensure even pigment retention: (1) Do NOT scratch, pick, or peel the scabs. Picking scabs will pull the pigment out, leaving blank patches. (2) Keep the area dry. Avoid splashing water on the treated area during showers for the first 7 to 10 days. Clean the area gently with a damp cotton pad as instructed. (3) Apply a very thin layer of the provided aftercare ointment twice daily using a clean cotton swab. (4) Do not apply makeup, skincare products (especially retinoids or acids), or sunscreen to the treated area until it is fully healed. (5) Avoid swimming, saunas, heavy sweating, and direct sun exposure for 2 weeks.

8. Optional Photography Authorization

[ ] I authorize the studio to take photographs of my treatment areas for my file and portfolio.
[ ] I authorize the studio to share before-and-after photographs of my treatment on social media.
[ ] I do NOT authorize any photography beyond internal record keeping.

9. Touch-Up and Retention Policy

PMU is a two-step process. The first session places the initial shape; a touch-up session is required 4 to 8 weeks later to fill in light patches, adjust color, and refine the shape. The cost of the touch-up session may or may not be included in the initial price. Skin type (oily skin retains pigment less effectively), lifestyle (heavy sun exposure, sweating), and aftercare compliance significantly affect pigment retention. No specific retention rate or lifetime is guaranteed.

10. Right to Refuse Service

The client has the right to stop the procedure at any time before the pigment is applied. The artist has the right to refuse service to anyone who appears intoxicated, or whose expectations are unrealistic. If stopped early, the client is responsible for paying for the sterile materials used.

11. Client Understanding and Questions

I confirm that I have read this document and understand the PMU process, the semi-permanent nature of the pigments, the risk of scabbing and color shift, and the strict aftercare rules. My questions have been answered.

12. Language Access Services

If English is not your primary language, a qualified interpreter is available at no cost. Please inform the studio before signing.

13. Copy of Consent Acknowledgment

I acknowledge that I have been offered a copy of this signed consent form.

14. Client Authorization

I voluntarily consent to receive the permanent makeup services described in this form. I authorize the shape mapping, pigment application, and use of topical numbing. I agree to follow the aftercare instructions and understand that touch-ups are required.
