Donor and Clinic Information

Nature and Purpose of Sperm Donation

Sperm donation is the process where a male provides semen samples to help individuals or couples achieve a pregnancy. The samples are frozen and stored (cryopreserved) for future use in intrauterine insemination (IUI) or in vitro fertilization (IVF) procedures. The donor must undergo a thorough screening process: (1) Complete physical exam and medical history review. (2) Semen analysis to check sperm count, motility, and morphology. (3) Infectious disease testing (HIV, Hepatitis B/C, Syphilis, Chlamydia, Gonorrhea) at the start and at regular intervals. (4) Genetic screening for inherited disorders. (5) Quarantine: all semen samples are frozen and kept in quarantine for at least 6 months before being released for clinical use, at which point the donor must be re-tested for infectious diseases.

Relinquishment of Parental Rights

I hereby relinquish all parental rights, duties, and obligations toward any child conceived using my donated sperm.
I understand that I will have no legal relationship to the children conceived, no rights to visitation, and no input in their upbringing.
I understand that I am fully protected from any financial liability or child support obligations for any children conceived through my donation.
I agree that the recipients of my donation have full legal authority and parental rights over any resulting children.

Anonymity and Identity Disclosure

[ ] Anonymous Donor: my identity will not be shared with the recipients or resulting children. Only my de-identified medical profile and physical traits will be shared.
[ ] Open Identity (ID Release) Donor: I agree that the clinic or sperm bank may release my identifying information (name, contact details) to resulting children once they reach 18 years of age, upon their written request.
[ ] Directed (Known) Donor: I am donating sperm to a specific recipient known to me. We have signed a separate Directed Donor Agreement detailing our relationship and expectations.

Material Risks and Disclosures

Privacy risk: despite strict privacy safeguards, the rise of commercial direct-to-consumer DNA testing (e.g. 23andMe, Ancestry.com) means that children conceived through donation can trace their genetic relatives, potentially identifying the donor in the future without the clinic's involvement.
Infectious disease detection: if my screening tests positive for an infectious disease, the clinic must notify me and, where required by law, report the result to the local department of health.
Genetic findings: genetic screening may reveal information about my own health or carrier status for inherited disorders. I will be informed of any significant findings.

Alternatives to Sperm Donation

Directed (known) donation instead of bank donation, which allows the donor to establish pre-defined boundaries with the recipients.
Choosing not to participate in the donor program.

Donor Compensation

Donor compensation is paid for the donor's time, travel, and adherence to the screening protocol, as detailed in the separate Donor Compensation Agreement. It is not a payment for the sperm itself. Compensation is only paid for samples that meet all quality and screening standards.

Right to Withdraw

The donor has the right to withdraw from the program at any time before his samples are released from quarantine for clinical use. Once a sample has been released, purchased by a recipient, or used in a procedure, the donor cannot recall or request the destruction of that sample.

Donor Understanding and Questions

I confirm that I have read this document and understand the sperm donation process, the legal relinquishment of parental rights, the anonymity settings, and the screening requirements. All my questions have been answered.

I acknowledge that I have been offered a copy of this signed consent form.

Donor Authorization and Agreement

I voluntarily agree to participate in the sperm donation program. I authorize the collection, freezing, quarantine, and clinical distribution of my semen samples. I confirm that all medical history and lifestyle disclosures I have made are accurate and complete.
