Informed Consent and Release for Tattooing Services

Client and Artist Information

1. Nature and Process of Tattooing

Tattooing is a form of permanent body modification in which an artist uses a hand-held machine with sterile, single-use needles to rapidly puncture the skin and inject pigment (ink) into the dermis (the second layer of skin). The process involves: (1) Skin preparation: shaving, cleaning, and disinfecting the target skin area. (2) Stencil application: applying a temporary design outline to verify placement and size. (3) Outlining and shading: puncturing the skin repeatedly to insert the pigments. (4) Cleaning and dressing: washing the area and applying a protective barrier (bandage or film). The procedure causes mild to moderate discomfort or pain, and minor bleeding.

2. Sterilization and Needle Safety Standards

All needles used during the procedure are sterile, pre-packaged, single-use, and will be opened in my presence.
All tubes, grips, and machine components are either disposable single-use or sterilized in an autoclave in compliance with local health department regulations.
All skin preparation, pigment cups, and barriers are single-use, and the artist will wear fresh nitrile gloves throughout the session.

3. Health Screening and Disclosures

[ ] I do NOT have hemophilia, diabetes, epilepsy, or any condition that affects blood clotting or healing.
[ ] I am NOT currently pregnant or breastfeeding.
[ ] I am NOT under the influence of alcohol, drugs, or blood-thinning medications (like aspirin) today.
[ ] I do NOT have allergies to metals (nickel), pigments, latex, or local topical anesthetics.
[ ] I do NOT have a history of keloid scarring or abnormal wound healing.

4. Short-Term Risks and Side Effects

Local infection: bacteria can enter the open skin punctures. Signs of infection include spreading redness, severe pain, swelling, warmth, or pus. Requires immediate medical evaluation.
Allergic reactions: allergic reactions to tattoo pigments (especially red, blue, green, and yellow) can occur during healing or years later. Symptoms include itching, bumps, or rashes.
Skin irritation and peeling: minor swelling, redness, and peeling (similar to a sunburn) are normal for the first 1 to 2 weeks.

5. Long-Term and Permanent Risks

Permanence: tattoos are a permanent modification. Complete removal is difficult, expensive, and may not be fully successful, often leaving scars or discoloration.
Scarring: abnormal healing or picking at the scabs can lead to raised scars (keloids) or loss of pigment.
Granulomas: small nodules or inflammatory bumps can form around the tattoo pigment, as the body treats the ink as a foreign substance.
MRI complications: rarely, tattoo pigments (especially those containing iron oxide) can cause temporary swelling, burning, or image distortion during an MRI scan.

6. Alternatives to Permanent Tattooing

Temporary tattoos: adhesive press-on decals that last 3 to 7 days.
Henna (mehndi): a plant-based paste applied to the skin surface, staining it for 1 to 3 weeks. Black henna should be avoided due to severe chemical allergy risks.
No tattoo: choosing not to modify the skin.

7. Detailed Aftercare Guidelines

To ensure proper healing and prevent infection, you agree to: (1) Leave the protective bandage or film on for the recommended time (usually 2 to 24 hours). (2) Wash the tattoo gently with warm water and mild, fragrance-free soap using your clean hands only. Do not scrub. (3) Pat dry with a clean paper towel and apply a thin layer of recommended ointment. (4) Do NOT scratch, pick, or peel the scabs. (5) Avoid swimming, hot tubs, baths, and direct sun exposure for at least 2 weeks. (6) Apply SPF 50 sunscreen to the healed tattoo in the future to prevent fading.

8. Optional Photography Authorization

[ ] I authorize the studio to take photographs of my tattoo for my file and for their portfolio.
[ ] I authorize the studio to share photographs of my tattoo on their website or social media pages. My face will not be shown unless authorized.
[ ] I do NOT authorize any use of my images beyond internal record keeping.

9. Payments and Touch-Up Policy

Payment is due at the completion of the session. A complimentary touch-up is offered within [specify time: e.g. 60 days] if the tattoo heals unevenly, provided aftercare instructions were followed. Touch-ups requested after this period, or those needed because aftercare instructions were ignored (such as picking scabs), will be billed at the artist's standard rate.

10. Right to Refuse Service

The client has the right to stop the session at any time. The artist has the right to refuse service to anyone who appears to be under the influence of drugs or alcohol, or who exhibits abusive behavior. If the session is stopped early, the client is responsible for paying for the portion of the work completed.

11. Client Understanding and Questions

I confirm that I have read this document and understand the tattooing process, the permanent nature of tattoos, the risk of infection and allergic reaction, and the required aftercare. All my questions have been answered.

12. Language Access Services

If English is not your primary language, a qualified interpreter is available at no cost. Please inform the studio before signing.

13. Copy of Consent Acknowledgment

I acknowledge that I have been offered a copy of this signed consent form.

14. Client Authorization

I voluntarily consent to receive the tattooing services described in this form. I authorize the use of the pigments, needles, and placement discussed. I agree to follow the aftercare instructions and accept full responsibility for the permanent modification of my skin.
