Bacteremia: in rare cases, bacteria may enter the bloodstream during or around the drainage procedure, causing symptoms of systemic infection such as fever, chills, or feeling very unwell. Patients with heart valve abnormalities, joint replacements, or suppressed immune systems are at higher risk and may be treated with antibiotics before or after the procedure.
Scarring: all skin incisions result in scarring. The degree of scarring depends on the abscess size, location, and how well the wound heals.
Nerve or vessel proximity: abscesses near major nerves or blood vessels (such as in the groin, axilla, or neck) require careful technique to avoid injury to nearby structures.

Wound Packing and Follow-Up Instructions

If packing (gauze strip) is placed inside the abscess cavity: (1) The packing must be removed or changed within 24 to 48 hours. The patient must return to this facility or see another provider for packing removal. (2) After packing removal, the wound should be gently cleaned with warm water. A dry dressing should be applied. (3) The patient must attend all follow-up appointments as scheduled until the wound is fully healed from the inside out. This may take 1 to 4 weeks depending on the size of the abscess. (4) Contact the clinic or return to the emergency department immediately if: the pain worsens after 48 hours, the wound shows increased redness, swelling, warmth, or pus, or if the patient develops a fever above 38 degrees Celsius (100.4 degrees Fahrenheit).

Alternatives to Incision and Drainage

Antibiotics alone: current clinical evidence shows that antibiotics without drainage are significantly less effective than I&D for most skin abscesses. Antibiotics alone are not recommended as the primary treatment.
Needle aspiration: some small, simple abscesses may be drained using a large-bore needle rather than a scalpel. This is a less invasive option but has higher rates of incomplete drainage and recurrence.
Warm compresses and observation: may be considered for very early, small abscesses that have not yet fully pointed. This is not recommended for formed, fluctuant abscesses.
No treatment: leaving a formed abscess untreated risks the infection spreading to deeper tissues (cellulitis or fasciitis), entering the bloodstream (sepsis), or burrowing to form a fistula tract. This is not recommended.
