What Is Functional Endoscopic Sinus Surgery (FESS)?

The sinuses are air-filled spaces in the bones around your nose and eyes. They are connected to the inside of the nose through small openings. When these openings become blocked due to infection, inflammation, polyps, or structural problems, mucus builds up and causes chronic sinusitis (long-term sinus inflammation). Functional endoscopic sinus surgery (FESS) is a minimally invasive procedure done entirely through the nostrils using a small telescope (endoscope) and fine instruments. No cuts are made on the outside of the face. The surgeon widens the natural openings of the sinuses, removes diseased tissue, polyps, or obstructing bone, and restores normal airflow and drainage. This usually requires repair under anesthesia.
Nasal septal injury: the wall between the two sides of the nose may be affected, causing a hole or deviation. This is uncommon.
Change in sense of smell: some patients notice a temporary change or, rarely, a permanent reduction in the sense of smell after sinus surgery. Smell often improves once inflammation is reduced.
General anesthesia risks: nausea, sore throat, allergic reactions, and rarely more serious complications. These will be discussed separately by the anesthesia team.

Alternatives to Sinus Surgery

No treatment: continued sinus infections and chronic inflammation with the risk of progression to more serious complications, including the spread of infection.

Recovery and Post-Operative Care

Recovery from FESS typically takes 1 to 2 weeks for most daily activities and 4 to 6 weeks for the sinuses to fully heal. During recovery: (1) Do not blow your nose for at least 1 week. If you need to sneeze, sneeze with your mouth open. (2) Use saline rinse (nasal irrigation) as directed, usually starting 24 to 48 hours after surgery. This is one of the most important things you can do to help healing. (3) Avoid strenuous activity and exercise for 2 weeks. (4) Avoid aspirin and ibuprofen for 2 weeks before and after surgery unless prescribed. (5) Attend all scheduled follow-up visits for endoscopic cleaning of the sinuses, which helps prevent scarring and supports healing. Missing these visits significantly increases the risk of poor outcomes. (6) Contact the surgeon right away if you have heavy bleeding, clear watery fluid draining from one nostril, severe headache, vision changes, fever above 38.5 degrees Celsius (101.3 degrees Fahrenheit), or stiff neck.
