What Is Pulmonary Function Testing?

Pulmonary function tests (PFTs) are non-invasive breathing tests that measure how well your lungs work. Coughing: forceful breathing efforts can trigger coughing, especially in patients with airway disease. Coughing may affect test results and additional efforts may be needed.
Chest tightness or shortness of breath: some patients feel temporary chest tightness during or after forced breathing efforts. This usually resolves within minutes.
Bronchospasm: rarely, the forceful effort itself or the methacholine challenge can cause significant airway narrowing. The technologist will monitor your breathing closely and a rescue inhaler is available at all times.
Bronchodilator side effects: if a bronchodilator is given, you may notice a faster heartbeat, mild shakiness, or nervousness. These effects are short-lived.
Physical strain: the test requires repeated forceful breathing. Patients with recent surgery, eye surgery, recent heart attack, or a known aneurysm may not be suitable for certain tests. Please inform the technologist of any of these conditions.

Conditions That May Affect Testing

[ ] I have had a recent heart attack (within 1 month), unstable angina, or recent chest surgery. Please inform the technologist before starting.
[ ] I have had recent eye surgery (within 1 month), including cataract surgery. Some tests that require increased pressure inside the chest may not be safe.
[ ] I have a known aortic aneurysm or another condition where straining is unsafe.
[ ] I have had a recent pneumothorax (collapsed lung) or lung surgery within the past 3 months.

How to Prepare for the Test

To get the most accurate results: (1) Do not use short-acting bronchodilator inhalers (such as albuterol or salbutamol) for at least 4 hours before the test unless your doctor tells you otherwise. (2) Do not use long-acting bronchodilators (such as salmeterol or formoterol) for at least 12 hours before the test unless instructed. (3) Do not smoke for at least 4 hours before the test. (4) Avoid large meals for at least 2 hours before the test as a full stomach can limit your breathing. (5) Wear loose, comfortable clothing. (6) Tell the technologist about all your current medications.

Alternatives

Chest X-ray or CT scan: imaging tests that show the structure of the lungs but do not measure how well they function.
Peak flow monitoring: a simple home device that measures the fastest speed you can blow air out. It is less accurate than spirometry but can be done daily at home.
No testing: if declined, your doctor will have less information to diagnose or monitor your lung condition. Treatment decisions may be less accurate as a result.

Right to Refuse or Stop

You may stop the test at any time during the session by telling the technologist. You may also choose not to have the test. Declining will not affect your ongoing care, but it will limit the information available to your doctor for diagnosing or managing your lung condition.
