Donor and Registry Information

1. Purpose and Process of Human Tissue Donation

Human tissue donation involves donating organs, eyes, skin, bone, or other tissues for transplantation, medical education, or scientific research. Tissues can be donated by a living person (such as bone removed during a joint replacement surgery, or skin from a reconstructive procedure) or after death (post-mortem). Donated tissues are processed, sterilized, and stored in a tissue bank. Transplanted tissues can restore sight, repair severe burns, reconstruct joints, or save lives. Tissues authorized for research help scientists study diseases and develop new surgical techniques.

I understand that my tissue donation is a voluntary, non-profit gift. Once harvested, the tissue becomes the property of the tissue bank. I waive any rights to the tissue and understand that neither I nor my family will receive any financial payment, compensation, or royalties if research using my tissue leads to a commercialized product, drug, or patent.

6. Next-of-Kin and Family Veto Parameters

Under the Uniform Anatomical Gift Act (UAGA), a person's documented decision to donate their organs or tissues after death is legally binding and cannot be overridden by family members or next-of-kin. However, to ensure a smooth donation process, the tissue bank strongly encourages you to discuss your donation wishes with your family so they are aware of your decision and will support the coordinator's efforts during a difficult time.

7. Regulatory Registries and Privacy

Your donation registration is recorded in a secure state or national donor registry. All personal information is protected by strict privacy laws (HIPAA). Coded database entries are used for logistics and matching. Identifying details will never be shared publicly.

9. Right to Revoke Registration

You have the right to revoke your tissue donor registration at any time. You can remove your name from the online donor registry, or submit a written cancellation request to the tissue bank. Revocation will take effect immediately. Tissues already donated and processed cannot be recalled.

10. Voluntary Participation Notice

Registering as a tissue donor is voluntary. Declining to sign this form will not affect your access to medical care, treatment quality, or insurance benefits. You can withdraw your name at any time without penalty.

11. Donor / Representative Understanding

I confirm that I have read this document and understand the human tissue donation program, the authorized uses (transplant vs. research), the medical screening requirements, the UAGA family veto rules, and my right to revoke. My questions have been answered.

12. Language Access Services

If English is not your primary language, a qualified interpreter is available at no cost. Please inform staff before signing.

13. Copy of Consent Acknowledgment

I acknowledge that I have been offered a copy of this signed consent form.

14. Donor / Representative Authorization

I voluntarily authorize the donation of human organs and tissues as selected in this form. I authorize the tissue bank to review my medical records, perform infectious disease testing, and harvest the authorized tissues as planned. I agree to the commercialization waiver.
