What Is a Thyroid Fine Needle Aspiration Biopsy?

The thyroid is a butterfly-shaped gland located at the front of the neck. It makes hormones that control many body functions. Thyroid nodules are lumps that form within the thyroid gland. Most nodules are benign (not cancerous), but some need to be tested to rule out thyroid cancer. A fine needle aspiration (FNA) biopsy is the most accurate and least invasive way to test a thyroid nodule. Thyroid ultrasound monitoring: a nodule with low-risk features on ultrasound may be monitored with repeat scans every 6 to 24 months instead of biopsy. This approach carries the risk of missing a cancer that grows between scans.
Thyroid nuclear scan: a scan using a small amount of radioactive material to look at thyroid function. It can identify nodules that are 'cold' (less functional, slightly higher cancer risk) or 'hot' (overactive, very low cancer risk). It cannot replace biopsy for diagnosis.
Surgical removal of the nodule: diagnostic surgery to remove part or all of the thyroid. This provides definitive tissue diagnosis but is a more invasive option usually reserved for cases where FNA is not possible or inconclusive.
No testing: if declined, a potentially cancerous nodule may go undiagnosed. Early diagnosis of thyroid cancer is associated with very high cure rates.

Preparation

No fasting is required before a thyroid FNA biopsy. If you take blood thinners such as aspirin, warfarin, or clopidogrel, inform your doctor several days before the procedure. You may be asked to stop them temporarily to reduce the risk of bleeding. Wear comfortable clothing with easy access to the neck area. You may drive yourself to and from the appointment.
