1. Nature and Process of Tattooing

Tattooing is a form of permanent body modification in which an artist uses a hand-held machine with sterile, single-use needles to rapidly puncture the skin and inject pigment (ink) into the dermis (the second layer of skin). The process involves: (1) Skin preparation: shaving, cleaning, and disinfecting the target skin area. (2) Stencil application: applying a temporary design outline to verify placement and size. 4. Short-Term Risks and Side Effects

Local infection: bacteria can enter the open skin punctures. Signs of infection include spreading redness, severe pain, swelling, warmth, or pus. Requires immediate medical evaluation.

Allergic reactions: allergic reactions to tattoo pigments (especially red, blue, green, and yellow) can occur during healing or years later. Symptoms include itching, bumps, or rashes.

Skin irritation and peeling: minor swelling, redness, and peeling (similar to a sunburn) are normal for the first 1 to 2 weeks.

10. Right to Refuse Service

The client has the right to stop the session at any time. The artist has the right to refuse service to anyone who appears to be under the influence of drugs or alcohol, or who exhibits abusive behavior. If the session is stopped early, the client is responsible for paying for the portion of the work completed.

11. Client Understanding and Questions

I confirm that I have read this document and understand the tattooing process, the permanent nature of tattoos, the risk of infection and allergic reaction, and the required aftercare. All my questions have been answered.

12. Language Access Services

If English is not your primary language, a qualified interpreter is available at no cost. Please inform the studio before signing.

13. Copy of Consent Acknowledgment

I acknowledge that I have been offered a copy of this signed consent form.

14. Client Authorization
